Optimal lung recruitment in acute respiratory distress syndrome remains controversial. Standard low and high positive end-expiratory pressure tables may not be superior to newer approaches such as driving pressure or electrical impedance tomography. While current guidelines discourage the use of aggressive recruitment maneuvers, is there still a role for this adjunct? This session will explore state-of-the-art approaches to these issues, including works in progress.

Learning Objectives:

  • Review current knowledge on the use of driving pressure to determine the best positive end-expiratory pressure (PEEP) in acute respiratory distress syndrome (ARDS)
  • Appreciate the importance of avoiding both underrecruitment and overdistension in patients with ARDS and the potential utility of nonaggressive recruitment maneuvers
  • Recognize the insufficiency of current guidelines for offering a personalized approach to PEEP in ARDS and discuss alternative approaches such as electrical impedance tomography
