Patients at extremes of age are poorly represented in clinical trials. Those of advanced age have been excluded because of multiple morbidities and disabilities. Overall, older adults account for less than 10% of all clinical trial subjects, and most older trial participants are healthier, with fewer comorbidities and concurrent medications than the average older adult requiring critical care. Selection bias, recruitment strategies, and other factors contribute to underenrollment of older adults in trials. This poses a threat to the external validity of these trials and complicates applying trial results to the patient population most likely to require care. In this session, the three cochairs of SCCM's new guideline on Caring for Older Adults in the ICU will discuss key considerations on the inclusion of older adults in critical care trials and provide insights into the data used to develop the new guideline.
